ABSTRACT

BackgroundBeing complex and highly heterogeneous with regard to the etiology and clinical manifestations of depression, neuroimaging studies make a breakthrough for exploring the biological subtypes of depression, while the current data-driven approach for the identification of subtyping depression using structural magnetic resonance imaging (MRI) data is insufficient. ObjectiveTo explore the biological subtypes of depression using diffusion tensor imaging (DTI) and machine learning methods. MethodsA total of 127 patients with depression who attended Beijing Anding Hospital from September 2017 to August 2021 and met the Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV) diagnostic criteria were included, and another 80 healthy individuals matched for gender and age were recruited through advertisements in surrounding communities during the same period. DTI findings, demographic characteristics and clinical data were collected from all participants. Tract-based spatial statistics (TBSS) and the Johns Hopkins University (JHU) white matter probability maps were used to extract fractional anisotropy (FA) values of white matter tracts. A semi-supervised machine learning technique was used to identify the subtypes, and the FA values for whole brain white matter of patients and controls were compared. ResultsPatients with depression were classified into two biological subtypes. FA values in multiple tracts including corpus callosum and corona radiata of subtype I patients were smaller than those of healthy controls (P<0.01, FDR corrected), and FA values in middle cerebellar peduncle, left superior cerebellar peduncle and left cerebral peduncle of subtype II patients were larger than those of healthy controls (P<0.01, FDR-corrected). Baseline Hamilton Depression Scale-17 item (HAMD-17) score yielded no statistical difference between subtype I and subtype II patients (P>0.05), while subtype I patients scored lower on HAMD-17 than subtype II patients after 12 weeks of treatment (t=2.410, P<0.05). ConclusionDepression patients exhibit two biological subtypes with distinct patterns of white matter damage. Furthermore, the subtypes respond differently to the medication treatment. ABSTRACT

ObjectiveTo explore the active ingredients, therapeutic targets, and relative signaling pathways of Tripterygium wilfordii in the treatment of triple negative breast cancer (TNBC) based on network pharmacology, and to verify the mechanism through in vitro cell model. MethodThe active ingredients of T. wilfordii were screened from the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP). The targets of TNBC were obtained from DisGeNET and GeneCards. Venny was used to identify the potential therapeutic targets of T. wilfordii against TNBC. Protein-protein interaction (PPI) network was constructed with String database. Gene ontology (GO) annotation and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment were carried out with DAVID to predict the mechanisms of potential targets. The molecular docking between triptolide and key targets were performed with AutoDock Vina. The effect of triptolide (0, 5, 10, 20, 30, 40, 50, 60, 80 nmol·L-1) on the proliferation of MDA-MB-231 cells was determined through methyl thiazolyl tetrazolium (MTT) assay. The effect of triptolide (0, 12.5, 25, 50 nmol·L-1) on the apoptosis of MDA-MB-231 cells was detected with Hoechst 33342 staining. Western blot was performed to detect the effect of triptolide (0, 25, 50 nmol·L-1) on the expression levels of key targets. ResultT. wilfordii had 23 active ingredients related to 55 potential targets of TNBC. GO and KEGG enrichment revealed that the potential targets were associated with 103 biological processes, 15 cellular components, and 35 molecular functions, and were involved in 140 signaling pathways including atherosclerosis and apoptosis. The results of molecular docking demonstrated that triptolide could bind with the targets including threonine kinase 1 (Akt1), vascular endothelial growth factor A (VEGFA), cellular tumor antigen p53 (p53), transcription factor AP-1 (JUN), signal transducer and activator of transcription 3 (STAT3), tumor necrosis factor (TNF), mitogen-activated protein kinase 8 (MAPK8), prostaglandin G/H synthase 2 (PTGS2), and Caspase-3. According to the results of MTT assay, triptolide (20, 30, 40, 50, 60, 80 nmol·L-1) inhibited the proliferation of MDA-MB-231 cells compared with blank control (P<0.05, P<0.01). Hoechst 33342 staining showed that triptolide (12, 25, 50 nmol·L-1) induced the apoptosis of MDA-MB-231 cells compared with black control (P<0.05, P<0.01). Western blot showcased that 50 nmol·L-1 triptolide down-regulated the relative expression levels of p-Akt, TNF-α, and VEGFA, while 25 and 50 nmol·L-1 triptolide up-regulated the relative expression level of p53 in a dose-dependent manner compared with the blank control (P<0.05, P<0.01). ConclusionT. wilfordii has multiple ingredients, targets, and pathways in the treatment of TNBC. It may regulate p53, VEGFA, TNF-α and other key targets to induce cell apoptosis and suppress angiogenesis and inflammatory response, which provides a scientific basis for the further investigation and clinical application of T. wilfordii.

ABSTRACT

TetR family transcriptional regulators (TFRs) are widely distributed in bacteria and archaea, and the first discovered TFR was confirmed to control the expression of tetracycline efflux pump in Escherichia coli. TFRs can bind DNAs and ligands. Small molecule ligands can induce conformational changes of TFRs, inhibiting or promoting TFRs to control target gene expression. Currently, TFRs have a wide variety of ligands, including carbohydrates, proteins, fatty acids and their derivatives, metal ions, and so on. Due to the diversity of ligands, TFRs regulate a wide range of physiological processes, from basic carbon metabolism and nitrogen metabolism to quorum sensing and antibiotic biosynthesis. On the basis of the recent studies in our laboratory and the literature, we review here the regulatory mechanism mediated by ligands of TFRs in primary and secondary metabolism, as well as the application of ligands for TFRs in the development of gene route and the activation of antibiotic biosynthesis.

ABSTRACT

Objective:To evaluate the characteristics of carotid plaque and the immediate outcomes after carotid artery stenting (CAS) in diabetic and non-diabetic patients by optical coherence tomography (OCT).Methods:Patients underwent CAS and OCT before and after operation in the Department of Neurology, Jinling Hospital from January 2014 to March 2019 were enrolled retrospectively. The clinical features, the characteristics of carotid plaque on OCT and the immediate outcomes after CAS were compared between diabetic group and non-diabetic group. The risk factors of stent malapposition were analyzed.Results:A total of 46 patients were enrolled. Their age was 64.02±8.32 years and 41 were males (89.1%). There were 20 patients (43.5%) in the diabetes group and 26 (56.5%) in the non-diabetes group. The proportions of atherosclerotic plaque with thin fibrous cap (40.0% vs. 7.7%; χ2=5.166, P=0.023), plaque rupture (55.0% vs. 23.1%; χ2=4.945, P=0.026) and macrophage infiltration (60.0% vs. 30.8%; χ2=3.930, P=0.047) in the diabetic group were significantly higher than those in the non-diabetic group. Multivariate logistic regression analysis showed that older age (odds ratio [ OR] 1.208, 95% confidence interval [ CI] 1.033-1.413; P=0.018), coronary heart disease ( OR 15.953, 95% CI 1.142-222.952; P=0.040), alcohol consumption ( OR 6.192, 95% CI 1.098-34.923; P=0.039) and lower systolic blood pressure ( OR 0.944, 95% CI 0.894-0.997; P=0.037) were independently associated with stent malaposition. Conclusion:Compared with the non-diabetic patients, carotid plaque in diabetic patients may be more unstable. Older age, coronary heart disease, alcohol consumption and lower systolic blood pressure were associated with stent malaposition after carotid stenting. OCT can reveal the characteristics of carotid plaque and the immediate outcomes after CAS, which can provide strong evidence for treatment decision.

Background@#Diabetes can complicate hypertension management by increasing the risk of cardiovascular disease (CVD) and all-cause mortality. Studies targeting diabetes detection in hypertensive individuals demonstrating an increased risk of diabetes are lacking.We aimed to assess the performance of hemoglobin A1c (HbA1c) and its cut-off point in detecting diabetes in the abovementioned population. @*Methods@#Data from 4,096 community-dwellers with hypertension but without known diabetes were obtained from the Study on Evaluation of iNnovated Screening tools and determInation of optimal diagnostic cut-off points for type 2 diaBetes in Chinese muLti-Ethnic (SENSIBLE) study; these data were randomly split into exploration (70% of the sample) and internal validation (the remaining 30%) datasets. The optimal HbA1c cut-off point was derived from the exploration dataset and externally validated using another dataset from 2,431 hypertensive individuals. The oral glucose tolerance test was considered the goldstandard for confirming diabetes. @*Results@#The areas under the ROC curves for HbA1c to detect diabetes were 0.842, 0.832, and 0.829 for the exploration, internal validation, and external validation datasets, respectively. An optimal HbA1c cut-off point of 5.8% (40 mmol/mol) yielded a sensitivity of 76.2% and a specificity of 74.5%. Individuals who were not diagnosed as having diabetes by HbA1c at 5.8% (40 mmol/mol) had a lower 10-year CVD risk score than those diagnosed as having diabetes (P = 0.01). HbA1c ≤ 5.1% (32 mmol/mol) and ≥ 6.4% (46 mmol/mol) could indicate the absence and presence of diabetes, respectively. @*Conclusions@#HbA1c could detect diabetes effectively in community-dwellers with hypertension.

ABSTRACT

Objective To explore the clinical efficacy of hidden blood loss after operation of proximal femoral nail anti-rotation (PFNA) for intertrochanteric fracture of femur of the elderly with Bazhen Decoction; To discuss the effects on short-term complications after operation. Methods Totally 87 patients with intertrochanteric fracture treated with PFNA were included in the study. The non-randomized controlled trial design was used to divide them into treatment group (44 cases) and control group (43 cases). Both groups were routinely given traction of the affected limbs and tibia, controlled hypertension, diabetes, anti-inflammatory analgesia, and low molecular weight heparin routine anticoagulation. The treatment group was treated with Bazhen Decoction, one dosage per day, continuous administration to 1 day before discharge. The bleeding volume, blood transfusion volume, hemoglobin, length of stay and short-term complications were observed in the two groups. Results The perioperative bleeding, volume and hospitalization time of the treatment group were all lower than those of the control group, with statistical significance (P<0.05). No patient needed blood transfusion in the treatment group, while two patients in the control group needed blood transfusion, with nine units. There was statistical significance between total blood transfusion volume in the two groups (P<0.05). There was no statistical significance in Hb level between the two groups 2 h after surgery (P>0.05). There was statistical significance in Hb level 12–24 h after surgery and 5 d after surgery between the two groups (P<0.05). There was no statistical significance in short-term complications between the two groups (P>0.05). Conclusion Bazhen Decoction can improve Hb level and reduce recessive blood loss and total blood loss of elder patients with femoral fractures treated with PFNA, which is also good for postoperative rehabilitation.

Object To develop an easy-assembled and-disassembled image diagnostic module to solve the problems of the tent-form field medical system in size, weight, mobility, accessories, protection and etc, which is of high references for combination and integration scheme of large-scale digital imaging equipment by adopting box instrumentation.Methods The module had its design executed with system modeling, structure simulation and CAD after its service requirements were analyzed. The design fell into the ones for overall framework, combination mode, packaging and buffer vibration isolation, ergonomics, box instrumentation, radiography accessories for vertical and horizontal positions, protection device and etc.Results The module was gifted with radiodiagnosis-related performances such as digital radiography, digital diagnosis,diagnosis reporting, compatibility to vertical- and horizontal- position radiography, and radiation protection during war conditions.Conclusion The module developed gains advantages in power, continuous radiography, assembling and disassembling, accessories and protection, and thus meets the requirements of the tent-form field medical system in mass casualties imaging diagnosis.

ABSTRACT

Objective Modern warfare is geographically complex and changeable.The lack of terrain adaptability restricts the popularization and application of rescue robots in the frontline war.To solve this problem,this paper designs a new longitudinal-arm suspension mechanism,and carries out principle analysis and simulation optimization.Methods The automatic dynamic analysis of the mechanical system(ADAMS)was used to establish the virtual suspension model. According to the condition of rescue robot parameters, appropriate constraints and objective functions were set up to optimize the length of the suspension rod and spring stiffness coefficient.Results The suspension structure parameters were optimized by experimental simulation.The single longitudinal-arm suspension could convert the small stroke of the helical spring shock absorber into the large stroke of the tire movement according to the comparison of the experimental data. Conclusion The research and design of single longitudinal-arm suspension are of great significance to develop its physical prototype and to select the helical spring shock absorber,playing an important role in promoting the development of the small wheeled rescue robot.

This experiment was to study the constituents of the roots of Viburnum setigerum through various column chromatographic techniques. Thirteen compounds were obtained and their structures were identified using chemical and spectroscopic methods as (7αH, 8'αH)-4, 4', 8α-trihydroxy-3, 3', 9-trimethoxy-7, 9'-epoxylignan (1), (7αH, 8'αH)-4, 4', 8α, 9-tetrahydroxy-3, 3'-dimethoxy-7, 9'-epoxylignan (2), alashinol G (3), alashinol F (4), (-)-secoisolariciresinol (5), (7R, 7'R, 8R, 8'S)-3, 3'-dimethoxy-7, 7'-epoxylignane -4, 4', 9, 9'-tetraol (6), (7αH, 8αH, 8'βH)-4, 4', 7'α, 9-tetrahydroxy-3, 3'-dimethoxy-7, 9'-epoxylignan (7), loganin (8), dihydroquercetin (9), protocatechuic acid (10), 4-hydroxy-3-methoxy-benzoic acid (11), adoxoside (12), and catechin (13). Compound 1 was a new compound. Compounds 3-7 and 11 were reported from the genus Viburnum for the first time. All compounds were separated from this plant for the first time.

ABSTRACT

Objective To design a visual terrain classification algorithm to facilitate the robot to make appropriate movement strategy by perceiving the surrounding environment.Methods Bag of words (BOW) and support vector machine (SVM) were used to develop a simple and effective terrain classification algorithm.The BOW model involved in feature extraction,codebook generation and feature coding.The mid-level feature developed by BOW model was then fed into SVM classifier to obtain the terrain classification result.Results The quadruped robot platform was applied to performing visual terrain classification experiment in the natural environment.The test environment included floor,asphalt,sand and grass.Good experimental results were achieved,and the classification accuracy was above 90% (the beat was 97.54% for grass).Conclusion The algorithm can effectively and accurately distinguish all kinds of terrains,with high accuracy and good stability.The key frame selection method needs researching in the future.

ABSTRACT

Triptolide is an epoxidated diterpene lactone compound separated from Tripterygium wilfordii. It has been reported that triptolide inhibits the growth of ovarian cancer cells in vitro and suppresses tumor growth in vivo. Triptolide could also sensitize ovarian cancer cells to cisplatin. Having combined the latest literatures on the effects of triptolide on ovarian cancer domestic and abroad, this article reviews the recent research advances in the anti-ovarian cancer activity of triptolide and its mechanisms which include inducing apoptosis of ovarian cancer cells, interfering in the cell cycle, and suppressing cell metastasis.

ABSTRACT

AlM: To discuss the application effect of surgical simulator to improve the microsurgical skills in junior ophthalmologist. METHODS: Lecture teaching, training in surgical simulator and operation in animal eyes were received in all these ophthalmologists. Results of the ability of operation in cataract surgery after this training were analyzed. RESULTS: After taught theory, students completed cataract surgical procedures on simulator and the mean test score was 75. 91 ± 6. 53 points. After trained on simulator repeatedly, the mean test score was 85. 57±4. 64 points. There was statistically significant difference ( P<0. 01) . During the third stage of practicing on animal eyes, the score was 89. 77 ± 7. 61 points, there was statistically significant difference compared with former two stages (P<0. 05).CONCLUSlON: Comprehensive training can improve microsurgical skills of junior ophthalmologist, but the long effect need to be observed.

ABSTRACT

To verify the effect of echinacoside on replication and antigen expression of hepatitis B virus (HBV) by using HBV-transfected HepG2. 2. 15 cells as the in vitro model. The ELISA method was used to determine HBeAg and HBsAg levels in cellular supernatants. The effect of echinacoside on HBV replication was studied by using HBV transgenic mice as the in vivo model. First of all, the HBV DNA level in hepatic tissues was quantified with PCR method. Meanwhile, the serum transaminase levels and hepatic pathological changes were also evaluated. Subsequently, HBV transgenic mice were divided into five groups: the control group, the lamivudine group (50 mg · kg(-1)) and echinacoside high, medium and low dose group (50, 25 and 12.5 mg · kg(-1)). The mice were orally administered with drugs once per day for 30 days. At the 31st day, the mice serum was separated to measure HBsAg, HBeAg and HBV DNA. Additionally, the liver HBV DNA level and histopathological change were detected. The results indicated that echinacoside at 50 and 100 mg · L(-1) suppressed significantly HBsAg and HBeAg expressions on the sixth day, with the maximum inhibition ratios of 42.68% and 46.29%; And echinacoside at 100 mg · L(-1) also showed an inhibitory effect on HBV DNA. Besides, echinacoside at 50 mg · kg(-1) inhibited significantly HBsAg and HBeAg expressions of HBV transgenic mice, with the inhibition ratios of 42.82% and 29.12%, and reduced markedly the serum HBV DNA level in HBV transgenic mice. In conclusion, the study suggested that echinacoside has a strong effect against HBV replication and antigen expression.

ABSTRACT

Objective To construct erythromycin-overproducing mutants by tandemly expressing S-adenosylmethionine synthetase gene metK, Vitreoscilla hemoglobin gene vhbS and pleiotropic regulatory gene adpA in Saccharopolyspora eryth-raea.Methods Through PEG-mediated protoplast transformation , the integrative plasmid carrying metK, vhbS and adpA was respectively introduced into erythromycin-producing wild-type strain S.erythraea A226 and industrial strain WB .The engineered strains were generated by apramycin resistance screening and PCR identification .The erythromycin production was compared in original strains and their mutants by the inhibition test of Bacillus subtilis and HPLC analysis .Results and Conclusion Four A226-derived mutants A226-P1-P4 and three WB-derived mutants WB-P1-P3 were independently obtained.Compared with wild-type strain A226, the relative erythromycin titer of the four engineered strains A 226-P1-P4 was increased from 8%to 25%by scoring the growth-inhibition zones .Further HPLC analysis showed that the four mutants had increased erythromycin A yield by 64%-94%.Likewise, the relative erythromycin titer and erythromycin A yield of the three engineered strains WB-P1-P3 were enhanced by 6%-10%and 31%-62%, respectively, in comparison with the original strain WB.The results show the universality of enhancing erythromycin productionvia tandem expression of metK, vhbS and adpA in S.erythraea.

ABSTRACT

Plateau environment has a wide range of diurnal temperatures and thin air. The surgical shelter needs to resist the huge temperature shock and work in harsh environments, placing higher noise immunity requirements on the microenvironment temperature control system. The traditional microenvironment control of surgical treatment shelters mostly use instrument display and switch control, with a high failure rate and functional dispersion. We have designed a plateau microenvironment temperature control system with PLC as the control center and the touch screen as the Human Machine Interface so that real-time monitoring and control of temperature is achieved and the plateau temperature shock is resisted while the heater fuel ratio can be adjusted properly to adapt to plateau environment. This paper describes the control system hardware, software and algorithm design in detail.

Objective To observe the changes of macular choroidal thickness before and after laser treatment for diabetic retinopathy patients.Methods For patients with diabetes by fundus fluorescein angiography (FFA) examination,diagnosed as severe non-proliferative and proliferative diabetic retinopathy and in accordance with the laser photocoagulation treatment indications of 23 patients (45 eyes) included in the study.There were 10 cases of male,13 cases of female; the average age was (55.48±5.43) years old.All patients underwent macular grid laser photocoagulation and pan-retinal photocoagulation.The macular choroidal thickness before and after laser treatment was measured by enhanced depth imaging technique of optical coherence tomography during follow-up at 1,2,3 weeks and one month at specific sites of choroidal.The specific sites included subfoveal choroidal thickness (SFCT),from the foveal 1mm,3mm,6mm distance of nasal choroidal thickness (NCT) and temporal choroidal thickness (TCT).Results One week after the treatment,SFCT,NCT1 mm,NCT3 mm,TCT1 mm,TCT3 mm,TCT6 mm were obviously thickening (t=4.728,4.422,3.759,3.743,5.713,2.502; P<0.05).Two weeks after the treatment the SFCT,NCT1 mm,NCT3 mm,TCT1 mm,TCT3 mm were decreased gradually (t =3.189,2.122,2.742,2.196,2.076 ; P<0.05).The choroidal thickness returned to pretreatment level from 2 weeks to 4 weeks after treatment,the NCT6mm had no obvious change in the whole treatment period(P>0.05).Conclusion The choroidal thickness was significantly thicker after laser photocoagulation treatment within 1 week,with the time prolonging the choroidal thickness gradually decreases.

ABSTRACT

During rescue operations in case of armed conflicts in cities , terrorist attacks and public health emergencies , rescuers are always in danger .For this reason, search for and rescue robots have been used to search and evacuate the wounded .They have also become a hot issue in current robotics technology research ,promising a wide range of applications in civilian and military fields .This paper outlines the origin of rescue robot research ,course of development and classifica-tion, focuses on the developments of and future research of the U .S army on evacuation and rescue robots ,and finally pres-ents the problems and key technical difficulties facing the development of such robots .This paper is interded to improve re-search methods and design theories of rescue robotics .

ABSTRACT

Objective To probe into characteristics of medical assurance for medical service preparation, medical staff deployment and injury treatment/rescue for martial arts competitions in Beijing Olympic Games. Methods Collecting data of all injuries treated onsite of martial art venues and in clinics of such venues, for statistical analysis of the injury occasions, injury location, injury types and post-injury treatment. Results During the test games, injuries found in martial arts repertoire competitions account for 0. 92% of all athletes in the category, mostly waist and lower limbs injury; injuries found in sanshou competitions account for 12.4% of all athletes in the category, mostly blunt trauma on head/face, body, perineum and four limbs, along with skin lacerations and 16 heavily wounded transferred to hospitals for rescue. No injuries are found in official games of martial arts repertoire competitions; in sanshou competitions, injuries account for 5. 5% of all athletes of the category, without heavy injuries for hospital rescue. Conclusion Athletes tend to be exposed to greater risk of injury in sanshou than in repertoire competition, headcount of injuries and injury heaviness in test games are higher than official games, probability of injuries of athletes of middle level is higher than those of other levels. Medical staff of professional surgeons of multidisciplinary skills are recommended.
